your card works: Present your Remedy Assurance Card
along with a $10 co-pay to Rapid Remedy Urgent Care
Center and you will be seen by a physician. Coverage
to include, but not limited to: |

/ Ongoing Management of Chronic Medical Problems

3 in house xray studies per year

| *Some
Procedures and services are not covered by membership:
medication (including injections), work injuries and
workman comp; off-site labs, life threatening events,
specialty x-rays, casting supplies, splinting materials,
crutches, IVs, chronic pain management, lacerations,
Medical problems deemed of a nature too severe or chronic
to be cared for at our facility.
Your
Membership card also entitles you to receive 20% discount
on all injections and onsite pharmacy medications.

Payment
options are as follows: Prepayment, semi or annual discounted
rates; monthly bank drafts or credit card. Monthly billing
is not an option. Semi/Annual payment withdrawals must be
made thirty (30) days prior to expiration date. Check draft
or credit card rejections will result in immediate membership
termination and forfeiture of card.
